Home-Based Care
Essential clinical services delivered directly to the patient's home by a verified registered nurse. Includes vital signs assessment, blood pressure monitoring, blood glucose testing, wound care, and basic diagnostics. All visits are conducted using DigiCare-issued standardised equipment and documented in the patient's digital health record on completion.
Doctor Access
Where a patient requires a physician assessment, a verified General Practitioner is available for a home consultation through the Apomuden platform. The doctor conducts a full clinical review, documents findings, and initiates referral or escalation where clinically indicated. Doctor home visits through Apomuden are priced below the equivalent cost of a private facility consultation.
Pharmacy Coordination
Where medicines are recommended during a home visit, patients can request fulfilment directly through the Apomuden app. The platform coordinates the order with the nearest verified partner pharmacy. Partner pharmacies operate under a formal platform agreement and provide delivery where capacity allows.
Laboratory Services
Where diagnostic tests are required, the platform coordinates with verified partner laboratories to arrange home sample collection or guided facility visits. Test results are returned digitally through the platform. Abnormal or critical values are automatically flagged to the clinical team, ensuring findings are reviewed without relying on the patient to act alone.
Preventive Care & Chronic Condition Monitoring
Apomuden is designed specifically for the ongoing monitoring and early detection of the chronic conditions most prevalent in Ghana: hypertension and diabetes. Regular structured visits build a clinical picture over time, identify deteriorating readings before they become crises, and create the continuity of care that informal and ad hoc health interactions cannot provide.
Pricing & Transparency
All service fees are shown to patients in full before booking is confirmed. The service fee and transport fee are displayed separately with a clear total. There are no hidden charges. Services eligible for National Health Insurance Scheme coverage are flagged at the booking stage. DigiCare is in active dialogue with the Ministry of Health on broadening NHIS alignment for community screening services.
