DigiCare's referral pipeline connects home-visit cases requiring escalation with verified health facilities. Partner facilities receive structured referrals — with accompanying digital patient records — rather than uncoordinated walk-in presentations. In Year 1 of the pilot, DigiCare will refer cases from 120 active providers across Greater Sunyani.

Pharmacy Partners

Partner pharmacies fulfil medicine orders generated during home visits through the Apomuden platform. Patients confirm their prescription within the app; the pharmacy receives a structured fulfilment request. Partner pharmacies benefit from a consistent, platform-generated order flow requiring no additional sales effort.

Laboratory Partners

Partner laboratories receive test requests from home visits coordinated through the Apomuden platform. Sample collection logistics are handled in coordination with the lab's preferred workflow — home collection or guided facility attendance. Results are returned to the platform and delivered digitally to the patient and attending clinician.

Platform listing, integration walkthrough, and process alignment completed within 2 weeks of agreement.

Go Live
