We exist to close the gap in community healthcare delivery.
DigiCare Health Solutions Limited is a digital health company incorporated in Ghana in March 2026. We develop and operate Apomuden — a Twi word meaning Good Health, and a statement of intent.

Who we are
DigiCare was founded to address a structural gap in Ghana's community healthcare delivery. Qualified nurses and doctors are underemployed or operating informally, while households managing chronic conditions lack access to structured monitoring, documented care, and clinical continuity.
Apomuden is the platform that connects both sides of that gap — bringing verified clinical providers directly to households through an on-demand dispatch system, while ensuring every visit is documented, accountable, and connected to what came before.
Vision
To enable accessible, connected, and accountable healthcare for every community through integrated service delivery, supported by digital infrastructure that works at scale.
Mission
To deliver primary healthcare through a coordinated network of verified providers, supported by a unified digital platform that ensures efficiency, clinical accountability, and continuity of care across every patient interaction.
The Challenge
Healthcare access in Ghana's secondary cities is delayed, fragmented, and disconnected. Patients cycle between providers without a shared clinical record. Chronic conditions are detected late. Facilities absorb the cost of preventable complications.
Nurses and doctors with the skills to address these conditions at community level lack the infrastructure to do so in a structured, accountable way. The result is a healthcare system that is clinically capable but structurally inefficient at the point where most care should be delivered: the community.
Delayed access to primary care in secondary cities
Fragmented services with no shared clinical record
Chronic conditions detected too late for early intervention
Qualified clinicians underemployed or operating informally
Facilities overwhelmed by preventable community-level cases
Our Approach
DigiCare shifts the point of care closer to the patient. Verified nurses and doctors are deployed to households equipped with DigiCare-issued diagnostic tools, operating under structured clinical protocols.
Clinically Designed
Protocols designed by Dr Kwaku Asiedu Adjei-Manu, Principal Medical Practitioner at Sunyani Teaching Hospital, governing every service delivered through Apomuden.
Provider-Centred
The model creates structured, flexible income streams for qualified nurses and General Practitioners through a per-visit earnings model paid directly to mobile money.
System-Complementary
DigiCare does not compete with hospitals. It reduces basic screening demand at facilities and ensures complex referrals arrive documented and prioritised.
